Seminary/Formation Program Annual Report Submission Portal

Obtain annual reports from the theological educational program of formation (III.8.5.j) or

from those authorized by COM to oversee the evaluation program (III.6.5.i). The seminary or other formation program must provide annual reports indicating your academic and personal progress. It’s your responsibility to request and provide the reports to you COM Contact and the Bishop's Assistant via this form for each year.
