Please use this form to register candidates for a service of Confirmation, Reception, and Reaffirmation. This should be completed by the parish priest, senior warden, or parish administrator. Please do not complete this form as an individual.
Names on this form will be printed on the candidates' certificates. Please be sure spellings are correct, and that you have correctly entered them as a candidate to be confirmed, received, reaffirmed, or baptized. If you are registering more than ten people, please submit multiple forms.
In order for diocesan staff and hosting parishes to properly prepare for each service, please observe the following registration deadlines:
- May 9th service at Trinity (Alpena), register by
April 24th
- November 22nd service at St John's (Saginaw), register by
November 9th
