Form 9.3: Certificate for Reception to the Priesthood - Canon III.10.3

This form is only for those individuals previously ordained in a different faith tradition who are seeking reception in The Episcopal Church as a Priest.

This form must be submitted by the clerk of the vestry of the parish at which the candidate is a member.

You must complete this form on paper and submit it online.

You'll upload a scanned copy (or clear photographed copy) of the form below. Please do not mail your paper copy to the diocesan offices. To do so risks a delay in processing.

Upon your completion of this form, it will be sent to the Commission on Ministry Contact for the candidate and the diocesan office where it will be added to the candidate's file. You will receive a copy of the completed form for your convenience.
