You'll upload a scanned copy (or clear photographed copy) of the form below. Please do not mail your paper copy to the diocesan offices. To do so risks a delay in processing.
Upon your completion of this form, it will be sent to the Commission on Ministry Contact for the candidate and the diocesan office where it will be added to the candidate's file and forwarded on appropriately. You will receive a copy of the completed form for your convenience.
