This form must be submitted by a member of the Standing Committee. It can be completed in one of two ways:

In order to complete this form entirely online, you'll need all signatories present to sign electronically as you submit.

Upon your completion of this form, it will be sent to the Commission on Ministry Contact for the candidate and the diocesan office where it will be added to the candidate's file and forwarded on appropriately. You will receive a copy of the completed form for your convenience.
